Exam room portraitSixteenth Street Community Health Center has provided quality health care, health education and social services on Milwaukee’s multi-cultural south side since 1969. The Center is recognized as a leader in the community, in terms of the excellent care provided as well as advocacy for public health issues.
 
In 2008 Sixteenth Street provided health care services to 27,118 people.  25,185 of these people received medical care at 105,690 individual appointments.  2,166 people received behavioral health services with 11,233 individual appointments.

Case managers, outreach and education specialists and eligibility assistance workers helped 3,566 people at 15,769 individual appointments deal with complicated medication plans, learn how to manage their chronic illness or enroll in programs or services for additional assistance.

Sixty-three percent of Sixteenth Street patients reported incomes below the federal poverty level and 36% were without health insurance.  85% of patients are Hispanic or Latino, 9% are White, 3% are African American, 2% are Asian and 1% unreported or other.  63% of patients were best served in a language other than English.

1,728 women received pre-natal care at Sixteenth Street and 893 deliveries were performed by Sixteenth Street providers.  69% of these deliveries were done by the Certified Nurse Midwives in Sixteenth Street’s women’s health department.  94% of these babies were at or above normal birth weight.
