Sixteenth Street’s Chronic Care Project Salud para Todos  ¡Querer es Poder! (Health for All – Believe and Achieve) completed its three-year project run in October 2008.

The overall goal of the project was to develop a family oriented, linguistically and culturally appropriate community based program to combat the negative health effects caused by diabetes, asthma and obesity.  Major grant support for the project comes from the  Medical College of Wisconsin’s Healthier Wisconsin Partnership Program.
